FOREST THEATER RENOVATION PROJECT: CONTRACT CHANGE ORDER NO. 6

Description
CHANGES TO CONTRACT PLANS AND SPECIFICATIONS.
Addition: Supply all labor and materials necessary to complete the following: Reference PCO #43 (Otto's tracking number). Summary of Work includes hydro jetting of both 6" and 4" storm line removing dirt/root build-up/debris. Ran jetter upstream and downstream to exit point 3-4 times. Cleared out catch basins shoveling out dirt/debris.
Agreed Lump Sum. $1,965.00
